Sessions are by appointment only, Monday through Friday. Schedule yourself an appointment online using the BOOK NOW button on the side of this page. Appointments changed or canceled with less than 24 hours notice will be charged the full cost of the missed session (illness and emergencies excepted).
My schedule can be quite booked up. To add your name to the Cancellation List add your email to the “join the cancellation list” form in the sidebar. You may also call me or email me to let me know if you are in dire need, and I can try to call you when I have a bit more time between the cancellation call and the appointment opening.
New clients please fill out the intake form that the booking system sends you and feel free to download this HIPAA form to fill out and bring with you, or email the completed version to email@example.com. Here is the Notice of Privacy Practices referenced within the HIPAA form.
Gift Certificates are available, priced the same as regular treatments through the BOOK NOW button.
I accept cash, checks, PayPal, Venmo (@Bevin-Keely), the “Cash” app ($BevinKeely), most major credit cards, and do work with some health insurance companies, as well as L & I and automobile insurance claims. If you have a Health Savings Account (HSA) or a Flexible Spending Account (FSA) my services may qualify as an eligible expense. There is a $5/session discount for payments with cash or check at time of service.
Insurance Reimbursement: Massage therapy can be billed through your insurance if it is considered to be medically necessary. For more details on using your insurance to pay for your time with me, please go here.
